Hilal grabbed an early lead through international striker Yasser Al-Qahtani before Diaa Haron leveled for Nassr nine minutes later.
Things got out of hand midway through the second half when the referee denied Nassr a clear penalty after Talal Al-Mashaal was brought down in the area.
With tempers flaring on the pitch, Libyan international Tarek Al-Tayeb netted Hilal's second-goal.
Al-Tayeb's strike only added to the tension as Nassr's players claimed the player was in offside position.
Chaos hit when the referee disallowed a last minute goal for Al-Mashaal for an offside. Television replays showed the player was clearly onside.
The decision provoked havoc as Nassr's fans started throwing missiles on to the pitch and play was resumed after Nassr's officials intervened to calm down the crowd.
The win maintained Hilal's position at the Saudi league summit with 24 points while Nassr settled in eighth place with 10 points.
Elsewhere, Ittihad Jeddah were held to a 1-1 draw by Al-Ittifaq. Milinko Asimovic scored Ittihad's goal in the 90th minute to cancel out Mohamed Al-Sahli's 81st minute opener.
On Saturday, Tunisian internationals Haykel Guemamdia and Khaled Badra scored a goal each to give Ahli Jeddah a 2-0 win over title holders Al-Shabab.
