The mercurial player didn’t play in Ahli's league final round win over Gaish, as well as the CAF Confederation Cup clash against Santos also due to injury.
"I'm suffering from abs inflammation, which prevents me from running properly," the Egypt man told Ahli TV.
"Unfortunately I won't play against Ismaili as I need more time to fully recover."
Ahli and Ismaili will undergo a league decider in Alexandria on May 24th, after both teams finished the competition as joint-leaders with 63 points each.
In the meantime, defender Ahmed Al-Sayed confirmed that he will be ready to face the Dervishes, refuting rumours that he will be suspended from the clash.
The 29-year-old was cautioned against Gaish, and many suggested it was his third league yellow card before Al-Sayed assured it was his first.
Egyptian Premier League regulations impose one-game suspensions on players who collect three yellow cards.
